Once you've created your questionnaire, you may want to send this to your patients.

To do this, click "Settings" then "Questionnaires" then click the "..." on the questionnaire you would like to use as circled below, then click "View questionnaire":

Your questionnaire will open in a new tab. Copy the URL for this questionnaire:

You will now be able to paste this questionnaire link anywhere you would like to use it. This could be on your website, in letters or emails or even in your appointment confirmation emails.

If you would like to use this link in the "Appointment confirmation email" (Click "Settings" then "Appointment reminder") for instance you will be able to copy and paste this link in the template.

If you wish for this URL to be a clickable link in your emails, you must use HTML tags in your email template as below:

Once you have amended your template, scroll down to the bottom of the page and click "Save". The next time your appointment confirmation email is sent out you patient will be able to click on this link and fill in your questionnaire. Your email will look similar to the below email:
